Certificate documents are required (original copies)：

• Be sure to bring one of the following certificates to apply for correction:
  1. Original ID card issued by government organizations.
  2. Household list, household register, or related household papers when Taiwan was governed by Japan.
  3. Certificate documents issued by different-level government institutes or legal groups.
  4. Court verdict, decree, certificate of notarization, and letter of punishment of prosecutors' office.
  5. Certificate of birth issued by public & private hospitals, or qualified midwife who delivered the birth.
  6. Certificate documents of graduation issued by all-level schools or military & police schools and varied training classes, groups, and teams.
• Apply for correcting items of the registration of household is limited to one time except for the wrong record by the abolished household staff.
